From the case, crown, it looks like a 3468

 

but I don't know for the printed markers.

I had a 3415 my Patek AD identified for me (via its local watchmaker). They can open the case to read the serial numbers.

You can even ask for an archive extract that mentions date of purchase, references etc... It costs the equivalent of 100 CHF.

I don't know if the bracelet has been added to the watch or if it is genuine.

It looks it is in a very nice condition.
