Patek Philippe: Spare Wheel

Thanks Steve for your fast reply. I had initially wondered about That wheel being for a centre second hand, but dismissed because so many movements didn't use it. So, what I described as a "click" is a bridge with a pinion hidden underneath and with maybe some sort of pressure spring built in? Thank

I have puzzled for years over this, so I'm finally going to ask! Most movements I see have all the wheels between the plates, but maybe 5% have a fairly large wheel on top, or outside, of the plates, usually with what, to my uneducated eye, looks like a click at right angles to the wheel. What does
