Go for the one that you love more... Design wise...

 

The 5146 is the traditional look, but the 5146G with the gray face has an almost too sporty look.  A good look nonetheless.  

Consider the 5396G which has the look of the old perpetual calendar Patek Philippe watches (3488, 2499, etc.).  

Personally, the 5396R or G would be the first Patek Philippe perpetual calendar I'd consider.  I was very close to procuring a 5146G.  But at the end, I decided I loved the 5396G (with the sector dial at the time) more.  Alas, today, I have neither.  

Patek Philippe is the inventor of Annual Calendars, first coming with the concept in 1995.  So it would definitely be the favorite among all brands with an Annual Calendar.  However, this Lange watch is actually a very beautiful watch and really has the underpinnings of a perpetual calendar (look at the AC and PC statistics, there are only about two dozen major part changes, and the AC has half a dozen fewer parts than the PC).  The Lange AC is essentially a PC with a few revisions.  

In terms of reliability; I find that Patek Philippe perpetual calendars do seem to have a few problems every now and then.  It's hard to say for the Langes, as sample size is much smaller, but I haven't heard of any problems.  Lastly, the quality of the Lange watch exceeds that of the Patek Philippe watch in many ways (double assembly, slightly better movement finishing, more gold in the case, etc.); but despite these quality advantages, you really have to go for the one that you love the most.  If there are two equally smart girls/guys, and you can only marry one of them, go for the one that looks better (aesthetically) to you.  

I personally really love that 5396, and I will likely procure one someday.
