I suspect damaged tooling used to fab the part...

 

Both end pieces have identical defects. Probably made in the same batch. Fairly easy to remedy (re-machine or file smooth) before it left the factory.

Since those end pieces bolt onto the main base plate, they are easily swapped out in the field. Should be an easy fix.
