It’s pretty simple.

 

The pushers on the left side of the case advance a timing minute hand that normally “hides” under the regular minute hand. The lower pusher advances the timing hand five minutes. The upper pusher advances the timing hand one minute. So, for example, push the lower pusher once and the upper pusher 3 times and the hidden hand advances 8 minutes. When the regular minute hand reaches the hidden minute hand you’ve hit your target time.

The gold pusher in the crown returns the hidden hand to its hiding spot under the regular minute hand.

Here you see the pushers on the left side of the case and the timing minute hand uncovered.

Here you see the gold pusher built into the crown. The gold pusher coordinates to the gold timing hand.
