I have no idea if a spacer ring is used, but I assume so: larger case + small movement normally makes spacer rings necessary. However, it might even be prossible that the case is so thick to house the movement without spacer.
The second question is easier to answer: If you look at the picture of the PanoMatic Center XL, you might have noticed the pusher at 7. this reveals that the cal. 100 is used to power that watch. Therefore, a relocation of the second hand is not necessary at all.
