You propose a good idea.
However, consider the consequences of your idea...
When Patek Philippe does re-design their movements, they will quite likely find methods to reduce costs, especially designing parts that require less assembly and finishing time.
So, remember, there are always side effects that may be undesired. Be careful what you wish for...
Regarding the 5123, I think you should procure one. Your reason why you dislike it is unfounded. The 5123 has a case of 38mm. The movement has a diameter 21.9mm. If you look at traditional watchmaking, you will realize that the smallest diameter of case this watch could probably fit into is approximately 32-34mm. Generally, a watch needs at least 5-6mm of space for case and other empty space on the outside of the movement. You state the movement is "not much more than half the diameter of the case" and that the movement could "easily go into a 25mm womens' watch."
