Sorry to kick up a relatively old thread, but I'm also a shoe fanatic and wanted to make one correction to your otherwise very informative post: Santoni does make full goodyear construction shoes in their "fatte a mano" line - in fact, I have several pairs. Even within "fatte a mano" there are different quality levels - the best are two or three rungs below JL Paris RTW in quality but for my taste more suitable for a trip to the clubs or wear with jeans when a bit of piquant style is needed.
Have you by any chance tried the MTO program by Cleverly? I would venture to say that Cleverley deserves consideration for anyone's RTW/MTO needs. I saw a pair of their benchmade shoes at Quinn's LA and found them fantastic (and much cheaper than JL Paris). In terms of their bespoke, it is on a par with just about anyone and I like the chisel toe.
Closer to Asia, Ryota Hayafuji has some great bespoke shoes that I've seen worn by senior Japanese execs.
