In my experience, my surgical colleagues were Apple watch or Seiko types, since they were always removing their watch for handwashing and concerned about loss or damage. Office types (neurology, cardiology, etc.) had more flexibility with their choices.

I noticed an ENT wearing a WG Blue Day Date the other day, some people get shy and guarded if you say anything...
Which reminds me of a story my Uncle told me, he was leaving a meeting and a young lawyer ran up to him all excited and asked him if he is wearing a Panerai and complimented him on the watch, depends I guess.

It’s happened to me a few times, I’m always liking to see what someone is wearing and depending on the situation will ask. It helps to be older, as I’m no threat to anyone.
If I spot a watch and can make a casual remark...
...I'm usually rewarded with at least a brief conversation. I'm usually wearing a watch too so that probably helps. I think watch people are like car people. Usually happy to engage someone who seems truly interested and knows at least SOMETHING about wat...
In my neck of the woods pretty much zero.
I will see some Rolexes but there are a lot of people wearing Rolex who aren’t enthusiasts.

Pure class! It is not very often that I find a watch enthusiast. Most high horology users I’ve meet are entrepreneurs who wear a fine watch as a status symbol and are not very much into horology. They aren’t also prone to talk much about their pieces and ...
Close to never. Has happened a few times, but not even once a year!

It is rare that I stumble upon another enthusiast
I've seen nice watches on guys but that certainly doesn't mean they are 'enthusiasts". What I see mostly is Rolex, Rolex, oh yeah, and a few Rolex. Most of the wearers couldn't tell you which model it is, so definitely not enthusiasts.

Are we dying breed? I hope not. I am gifting only watches to my friends and family. I gifted apple watches a while ago but now only mechanical watches and the reaction is amazing, every one loves their watches. And makes me happy too, win win.
Not rare for me. In fact a couple of weeks ago at a cars n coffee I met a nice and very interesting guy with a cool 68 Mustang convertible. I noticed a Sea Dweller on his wrist
and commented on it. Turns out he was born in France but then partially grew up in Switzerland and has a nice collection. He later moved to the US, joined the Navy and ended up becoming an F-18 pilot! That explained why he had a number of Breitlings.
While in Miami, I saw several high-end pieces weren’t by individuals.
Not sure if it was just for the glamour or if they were true connoisseurs
